FlexStaff is hiring a Clinical Assessment Manager – Care Delivery (UAS Nurse) to join our client’s team on a temp to perm basis.
Location: Brooklyn (Orientation in the Bronx)
Setting: Field; conducting in-home participant assessments.
Pay Rate: $57/hr
Schedule: Monday – Friday 8:30am-5:30pm, (1 hour lunch) (FT)
We are seeking a Clinical Assessment Manager with required UAS experience to oversee and execute all tasks related to the clinical assessment process. This includes, but is not limited to, clinical onboarding, reassessment, UAS-NY completion, comprehensive assessment, medication review, and post-hospital discharge evaluation. The role involves close collaboration with intake, enrollment, and care delivery teams, conducting in-home assessments, ensuring accurate documentation in EMR, promoting the PACE program, and maintaining compliance with CMS/PACE guidelines. This individual will play a crucial part in facilitating smooth transitions for participants, advising on operational improvements, and actively contributing to quality improvement initiatives.
Here are the key responsibilities for this role:
Assessment & Eligibility:
- Conduct in-home or virtual assessments using the Uniform Assessment System of New York (UAS-NY) for prospective or enrolled participants.
- Determine initial or continued plan eligibility in accordance with Community Based Long Term Care (CBLTC) Services requirements.
- Answer questions about the PACE program and highlight its benefits during assessments.

Maintain a schedule of at minimum 2-3 UAS-NY assessments per day.
Documentation & Compliance:
- Evaluate and review previous assessments, primary diagnoses, and medications, ensuring all changes are accurately reflected in the EMR and match UAS entries.
- Document all interactions, notes, service plans, and other documentation within 48 hours.
- Work with the quality review team to ensure accuracy and completeness of documentation in both the Health Commerce System and the EMR.

Maintain knowledge of all applicable CMS/PACE guidelines and organizational policies.
Collaboration & Communication:
- Collaborate closely with intake, enrollment, and care delivery teams.
- Escalate significant changes or concerns during assessments to the quality review team or discipline manager.
- Provide significant clinical information to the Care Delivery team in advance of enrollment for smooth transitions.
- Partner with Care Delivery leadership and the Interdisciplinary Team (IDT) to provide updates, address concerns, and improve processes.
- Serve as the clinical point person for Care Delivery questions post-enrollment.

Remain responsive and available by phone/email for CenterLight staff.
Program Promotion & Education:
- Promote and educate potential referral sources about the PACE program.
- Educate potential/new PACE participants on plan benefits, services, and the care delivery model.
Process Improvement & Quality:
- Complete and/or reassess PCAT Tasking Tool, providing recommendations for modifications.
- Utilize the Lenavi analyzer during review and assessment processes.
- Engage in continuous quality improvement activities to promote optimal staff performance and participant outcomes.
- Advise on operational process improvements related to enrollment, scheduling, financials, reporting, and engagement strategies.
- Regularly review key performance metrics and analytics.
Logistics & Professionalism:
- Must be able to work some weekends and travel to various boroughs.
- Act only within the scope of individual’s authority to practice.
- Meet a standardized set of competencies for the position.
- Serve as an acting member of the IDT.
