Home Care Field RN – Manhattan

Schedule: M-F, 8-hour shifts
FlexStaff is seeking an experienced Registered Nurse for a Field RN position for our client, a leader in home health care. This position will cover cases in Manhattan.
This position will manage all aspects of patient care related to services provided in the home.
Responsibilities:
- Conduct home care assessments
- Perform home visits
- Complete all required documentation
- Communicate effectively with Primary Care Physicians (PCPs)
- Update medication profiles
- Coordinate care with other providers
- Review and complete case communication notes
- Participate in discharge planning activities
- Other duties as assigned
Requirements:
- Active New York State (NYS) RN License
- Home Health Care experience preferred
- Valid driver’s license and reliable transportation
