FlexStaff is seeking a Home Care Registered Nurse to work for one of our external clients, a non-profit organization that provide and manages long-term healthcare services, to conduct home care visits.
We have three openings for the below designated areas:
- Manhattan and Bronx, NY
- Bronx, NY
- Queens, NY
In this role you will be conducting routine visits to perform wound care, administer medications, and complete general health assessments.
Must have a car and a valid NYS driver’s license. Mileage reimbursement.
This is a permanent, full-time job. Schedule: Monday – Friday 8:30 AM – 5:30 PM, 37.5 hours per week. Available for weekends/nights coverage as needed.
An ideal candidate would have a nursing degree, a valid NYS RN License and minimum of three (3) years of clinical nursing experience in a certified home health agency (CHHA), acute care, medical surgical, and/or critical care experience.
Responsibilities include, but not limited to:
- Conduct home visits to provide clinical services, including wound care, medication administration, and health assessments.
- Deliver nursing care based on individualized care plans and physician orders.
- Monitor patient conditions and communicate changes to the care team and physician.
- Collaborate with partner organizations and care coordinators to address patient needs.
- Educate patients and caregivers on condition management, medication adherence, and home safety.
- Document care accurately in the electronic health record system.
- Adhere to clinical protocols, regulatory standards, and infection control guidelines.
- Participate in interdisciplinary case conferences and clinical meetings as needed.
