Responsibilities:
– Conduct routine home visits to provide wound care, administer medications, and perform health assessments.
– Develop trusting relationships with patients and caregivers, offering education on condition management, medication adherence, and safety.
– Monitor patient conditions, document care using our state-of-the-art electronic health record system, and communicate updates promptly.
– Collaborate with care teams and partners to ensure holistic, effective care tailored to each individual.
– Travel throughout your service area, bringing expert care directly where it’s needed most.
Qualifications:
– An RN with a BSN from an NLN-accredited nursing program, with at least 5+ years of healthcare experience.
– Strong clinical judgment and the ability to work independently while maintaining team collaboration.
– Excellent communication skills for patient education and interdisciplinary coordination.
– A commitment to safety, infection control, and regulatory compliance.
– Flexibility and a passion for improving lives in the comfort of their homes.
